Seeing Before Treating: Why Ultrasound Has Changed the Way I Practice Aesthetic Medicine

When a patient comes in for an aesthetic treatment, they often ask me a very simple question: "Is it safe?"

For many years, aesthetic medicine relied mainly on anatomical knowledge derived from textbooks. Today, we have an additional advantage: the ability to see each patient's real anatomy before performing a treatment. As a radiologist specialising in ultrasound applied to aesthetic medicine, I consider ultrasound one of the most valuable tools for improving both safety and personalisation in aesthetic procedures.

I remember a patient who wanted to improve her facial profile. She had undergone several aesthetic treatments over the years but could not recall exactly which products had been used or where they had been injected. On clinical examination, her face appeared completely normal. However, ultrasound revealed the presence of previously injected material, allowing us to identify its location and assess the surrounding tissues. This information completely changed the treatment plan. Instead of immediately proceeding with new injections, we carefully evaluated her facial anatomy, assessed the existing volumes, and designed a more appropriate and safer approach. This is one of the aspects of aesthetic ultrasound that fascinates me the most: it allows us to see what is invisible from the outside.

Many patients assume that all faces are anatomically similar. In reality, this is far from true. Blood vessels may follow different paths from one individual to another. Previously injected fillers may be located in unexpected areas. Even the thickness of the skin and subcutaneous tissues can vary significantly between patients. Ultrasound enables us to identify these individual differences and tailor treatments accordingly.

Its role, however, goes far beyond prevention. Ultrasound is also extremely valuable during follow-up, as it allows us to monitor treatment outcomes over time and objectively evaluate how tissues respond. In addition, when issues such as persistent swelling, nodules, or vascular complications arise, ultrasound can help provide an accurate diagnosis and guide the most appropriate corrective treatment.

I believe the future of aesthetic medicine is not simply about doing more procedures—it is about doing them better. For me, doing better means understanding each patient's anatomy, personalising every treatment plan, and using technologies that allow us to see before we intervene. Ultrasound represents exactly that: a more informed, more precise, and ultimately safer approach to aesthetic medicine.

And when patients ask me whether a treatment is safe, my answer is always the same: "Safety begins with knowledge. Ultrasound allows us to know much more."
